New Influenza Like Illness MedicalTemplate for Flu Season 2009-10

The creators of MedicalTemplates have a new template designed specifically for the evaluation of patients with Influenza Like Illness (ILI).

The 2009-2010 influenza season has been marked by rapid spread of a novel H1N1 Influenza A virus (often referred to as “Swine Flu”). Identified in April 2009, H1N1 has been identified worldwide and is widespread in virtually every state in the United States as of the week ending October 17, 2009, according to the Centers for Disease Control and Prevention. In response to the rapid spread of this H1N1 virus, President Obama declared a national emergency on October 24, 2009.

Unlike seasonal Influenza A, the novel H1N1 Influenza A virus has affected primarily people under the age of 65. While a monovalent vaccine free of adjuvants has been created, it is not yet widely available. Groups targeted to receive the H1N1 Influenza A vaccine are

1. Pregnant women

2. People who live with or work with infants less than 6 months of age

3. Health care and emergency service personnel

4. Children and young adults ages 6 months-24 years

5. Adults aged 25-64 years with medical conditions that predispose them to influenza-related complications

The unseasonably early and rapid spread of influenza has resulted in an 8-fold increase in the number of outpatient clinic visits for Influenza Like Illness, further straining health care resources. By week 40 of 2009, 8% of all outpatient visits were due to Influenza Like Illness, compared to 1% in years 2006-2008 during the same period (CDC).

The Influenza Like Illness evaluation template can be used in either the outpatient or the inpatient setting by any health care provider.

The Influenza Like Illness evaluation template can be used for the evaluation of Influenza Like Illness from any cause, including seasonal influenza, novel H1N1 influenza, pneumonia from any cause, etc.

Prompters in the History of Present Illness (HPI) section assist health care providers in rapidly evaluating symptoms and risk factors for influenza, and permit ready documentation of recent vaccinations. The Past Medical, Social and Family History (PMSF) section has been streamlined to permit documentation of the most frequent chronic medical problems, functional status, risk factors and family history. The Exam section of the Influenza Like Illness evaluation template contains prompters for both pertinent positive and negative exam findings, and more space has been allocated for additional exam findings. The Impression and Plan section now has more space for documentation, and contains prompters for documentation of “code” status and Health Care Power of Attorney discussions.

COPD documentation just got easier!

MedicalTemplates has expanded and updated the COPD Evaluation template to include more of the information health care providers need to document.

Improvements include prompters for assessing COPD exacerbations and streamlining the Past Medical, Social and Family History. Other improvements include expanded exam findings suitable for either a general multipurpose exam or a single organ respiratory exam, space for ventilator settings, intravenous medications, and labs.

The Impression and Plan section is now expanded to an entire page to permit additional documentation for complex patient encounters.

MedicalTemplates’ new COPD documentation template decreases documentation time, increases health care provider efficiency, and effortlessly assists with improving encounter documentation.
